Almost Free Medical Check-ups (just $5 co-payment) The Motion Picture & Television Industry Fund continues to offer Comprehensive Physical Exams (CPE) free of charge (after the $5 co-payment) once every 12 months. This service is intended to assess your overall health and identify any unhealthy risk factors that might affect your health in the future. It is no longer necessary to request a voucher in advance. Just contact any MPHW clinic to schedule an appointment.

Predesignation of Personal Physician When the California Workers' Comp Laws were amended, Organized Labor worked hard to maintain the Worker's right to choose their own doctor in the event of on-the-job injury... otherwise, the Employer gets to pick your doctor. To retain your right to choose your own doctor, the Predesignation of Personal Physician Form must be submitted to your employer each time you start a job. Don't forget to keep a copy for your records.
